Dr Rozina Ali is an internationally recognised, multi-award-winning consultant plastic, reconstructive and aesthetic surgeon with more than three decades of experience. Known for her refined, detail- driven approach, she specialises in subtle, intelligent treatments that enhance, sculpt and elevate the natural face.
In the first blog in this series, Dr Ali introduced her layered approach to facial rejuvenation. In the second, she explored the importance of hydration and skin quality. In the third, she focused on rebuilding structure, restoring volume and balance. In this fourth blog, she turns to refinement, the stage where results are tightened, sculpted and elevated into something truly polished and natural.
Why refinement is where results come to life
Once the skin is healthy and the underlying structure has been restored, the final stage of in-clinic facial rejuvenation is not about doing more, but about doing less, precisely. This is where subtle adjustments create meaningful change.
Small pockets of excess fat, areas of heaviness, skin laxity or fluid retention can all soften definition and blur the natural contours of the face. Addressing these elements allows us to refine the
outcome, creating a face that looks more lifted, more defined and more harmonious, without appearing treated.
Sculpting and contouring: reducing what is no longer needed
Not all rejuvenation is about adding volume. In many cases, it is about carefully reducing excess. Fat dissolving injections are one of the ways I address small, localised pockets of fat that are resistant to diet and exercise, particularly under the chin or around the jawline. By breaking down fat cells, which are then naturally eliminated by the body, we can refine contours and restore definition in a very controlled way. The aim is not weight loss, but precision contouring.
When used appropriately, these treatments can subtly sharpen the jawline and improve facial proportions.
Similarly, botulinum toxin can be used as a sculpting tool, particularly in the lower face. Treatment of the masseter muscles allows me to soften jaw width, reduce heaviness and create a more balanced, tapered appearance. In my practice, I use botulinum toxin with a “little and often” philosophy. It is never about freezing the face, but about refining movement, improving balance and enhancing natural expression.
Tightening and lifting: restoring skin resilience
As we age, the skin loses its ability to contract and hold its shape. Even with good structure beneath, laxity can remain. This is where I often introduce energy-based treatments as part of a broader plan. Radiofrequency treatments work by delivering controlled heat into the dermis, stimulating fibroblasts to produce collagen and elastin. Over time, this improves skin firmness, tightens tissue and enhances overall tone.
Technologies such as Sofwave allow us to deliver ultrasound energy at precise depths, stimulating collagen production while protecting the skin surface. The treatment is comfortable, requires little to no downtime, and results continue to develop gradually.
Exion Face is another technology I offer within this stage of treatment, combining radiofrequency and ultrasound to tighten skin while also stimulating the body’s natural production of hyaluronic acid. This dual effect improves both firmness and hydration, creating skin that feels stronger, smoother and more luminous.
These treatments are most effective when the skin has already been optimised, which is why they sit naturally within the final stages of a layered approach.
Enhancing definition through circulation and drainage
Refinement is not only structural. It is also functional. Fluid retention, poor lymphatic drainage and sluggish circulation can all contribute to puffiness, heaviness and a lack of definition in the face. Even well-executed treatments can appear less effective if these elements are not addressed.
Manual lymphatic drainage is something I often incorporate, particularly around procedures, to support healing, reduce swelling and improve tissue health. It helps restore clarity and lightness to
the face.
Mechanical lymphatic treatments, such as Endoluxx Pro, can further support circulation and detoxification. By stimulating both lymphatic flow and blood supply, these treatments help refine
contours, reduce puffiness and improve overall skin tone.
The finishing touch: subtle, natural refinement
At this stage, the face is not transformed. It is refined. The skin is healthier, the structure is supported, and the contours are balanced. What remains is to enhance the overall quality, the luminosity, the clarity, the sense that someone looks well.
In many cases, this is about careful maintenance and small, considered adjustments over time. The most successful outcomes are often the least obvious. People notice that you look fresher, more rested, more confident, without being able to identify exactly why.
This stage completes the in-clinic aspect of the layered journey. Hydration and skin quality create the foundation. Structural restoration rebuilds support. Refinement then brings everything into
balance, tightening, sculpting and enhancing the final result.
In the final blog in this series, I will step outside the treatment room to explore the role of skincare, daily habits and long-term maintenance, the elements that protect, prolong and optimise the results we have created.
For me, facial rejuvenation has never been about a single treatment or a single moment in time. It is an ongoing, intelligent process that evolves with you. When we approach ageing in this way, thoughtfully, progressively and with respect for the individual, the results are not only more natural, but more enduring.
