As the year draws to a close and we look ahead to the possibilities of 2025, few voices carry as much authority in the world of aesthetic and reconstructive surgery as Dr Rozina Ali. An award-winning consultant plastic surgeon with over 25 years of NHS experience and three decades as a plastic surgeon, Rozina is internationally recognised for her expertise in breast surgery and her pioneering approach to natural results. Patients trust her for her blend of artistry, surgical mastery and caring, patient-centred practice.
In this blog, as we approach 2026, Dr Ali reflects on what she believes women are seeking in breast augmentation today, the technologies shaping the future of surgery, and her philosophy of balance and proportion that ensures results which feel as authentic as they look.
As an aesthetic and reconstructive plastic surgeon, I have witnessed breast augmentation evolve dramatically. Women are no longer focused on the largest possible implants or exaggerated results. Instead, they are asking for breasts that look soft, natural and in harmony with their body frame. I often describe the female chest as an ever-changing landscape, reflecting hormonal shifts, life stages and health. Surgery, when approached thoughtfully and in partnership, can restore balance, confidence and joy.
The wants and wishes of today’s patients are very different from those of even a decade ago. Gone are the days of rigid, high implants that looked more artificial than elegant. Modern patients want breasts that move naturally with the body, that complement their figure, and that feel authentic both in and out of clothing. A bra or basque can create dramatic uplift for a night out, but once undressed, it is natural softness and proportion that truly matter.
One of the key developments enabling these results is the next generation of breast implants. I am proud to offer Motiva Ergonomix™, a remarkable innovation designed with a soft dynamic gel and reinforced shell that allows the implant to behave just like natural breast tissue. I often place these implants over the muscle, directly beneath the breast gland. This not only reduces pain and bleeding, but also avoids the risk of animation deformity, where the implant moves with chest muscle exertion. The result is a beautifully shaped natural slope at the top of the breast and fullness exactly where it belongs, adapting fluidly whether the patient is standing, lying down or in motion.
Alongside implants, fat transfer is transforming the way I approach breast enhancement. With this procedure, live fat cells are harvested from areas of excess; filtered and prepared for injection into the breast envelope to create volume and contour. This technique allows for subtle enlargement, correction of asymmetry and, when combined with smaller implants, provides softness and cover that makes results almost indistinguishable from nature. Because fat transfer can be repeated in stages, it also offers the option of gradually building volume over time. I often describe fat as the “elixir of youth” because it not only adds volume but also improves the quality of the overlying skin, creating elasticity, lift and pliability that is not possible to replicate by any other means.
Choosing the right “look” is perhaps the most fundamental and personal decision. It is not simply about asking for a cup size, but about creating proportion across the chest, shoulders and waist. I place great emphasis on balance, symmetry and shape. To help patients make confident choices, I use advanced 3D imaging technology. While my years of experience enable me to visualise likely results instinctively, being able to show patients different implant types, sizes and positions on their own body, in real time, is invaluable. It transforms decision-making from a leap of faith into an informed, considered partnership….and a slightly less scary leap of faith!
Recovery too has benefited from innovation. Specially designed graduated compression garments from trusted brands such as Lipoelastic® and Macom® provide comfort and support in the critical early weeks. They stabilise and protect the breasts without underwires, allowing the body to heal while patients return to daily life.
Ultimately, my philosophy remains the same: breast augmentation is a sophisticated procedure to enhance balance, proportion and create results that feel comfortable, elegant and authentic. Shape is paramount, but so too is the feel of the breast. Process is paramount, listening to my patients, understanding their goals, and guiding them towards the most suitable options.
The future of breast augmentation is natural. With advanced implants, fat transfer, 3D visualisation and personalised aftercare, we can now achieve results that are beautifully proportioned, soft to the touch and lasting in their impact. I am proud to combine surgical artistry with the very latest technology, ensuring that every patient feels informed, empowered and delighted with their results.
